Because I'm ignorant:
1) Do all states do provisional ballots?
2) Do all states require you be given one if you are turned away, or are there additional conditions in some places?

@Retreival9096 Great question! BallotPedia explains the process. Idaho, New Hampshire and Minnesota are the only states without Provisional Ballots. You want to have a receipt as that allows you to be part of the "curing" process after the fact. Elections have been decided by "cured" ballots. https://ballotpedia.org/Provisional_ballot_laws_by_state
