The short version is everybody was very nice. They printed out a paper form so I could amend it and sign it and then they scanned it in The computer system was so actively hostile to this workaround though that none of the allergies and medicines associated with my record carried over, so the medical assistant and I had to enter them all from scratch in the exam room
