This is a FastPoint Technologies (Formerly FTG) PXL-2000.
It is a USB light pen interface.
If you never thought USB and light pens would be mentioned in the same sentence together, well, here we are.

Your light pen plugs into the front RJ-14 jack and your sync cable - not included, sadly - plugs into the back to tell the unit when HSYNC and VSYNC occur, which gives it the pen position. I assume it was not originally nicotine brown. This might be the first thing I ever retrobrite.
