Might be because humans on average settle with "good enough". Eyes + mouse work well enough and learning another scheme takes you through a valley of being worse before getting better. Many examples come to mind independent of accessibility.

For sighted users, scrolling and mouse-clicking pushed out learning keyboard shortcuts pretty quickly. I know people who don’t even know about tabbing through a form. Similar psychology? It does not help that operating systems and applications never cooperated very well on either standard shortcuts, or being able to remap them. @gins@mastodon.gamedev.place @WeirdWriter@caneandable.social
