Excellent example here. French railroad decided to skip some stops. A passenger who wanted one of those stops pulled the e-brake and exited the train.
Engineer botched the process of resetting the brakes.
Railroad was now way behind, skipped more stops.
Train came to the end of the line with no brakes. Many people killed. There were a few more errors too.
No air check was performed. An air check - continuity all the way to the last car - should be performed.
