Good questions, but that's as far as I got with the version control history. I'm presuming that there are some public discussions somewhere.
@daztucker@infosec.exchange is the person who made that commit marking the 'hack', so let's see whether xe is around. (-:

@JdeBP@tty0.social @ska@social.treehouse.systems @daztucker@infosec.exchange Solaris (which is where PAM came from originally) has some additional PAM related patches https://github.com/oracle/solaris-userland/tree/master/components/openssh/patches some of these are bug fixes and some add functionality to control the PAM service name based on userauth and a prefix. There are comments in them to explain why we have them and if they have been offered upstream.
